NMR-based metabolomics in kidney and lung disease


Chronic kidney disease (CKD) is a growing problem across the world, mainly due to increasing rates of high blood pressure and diabetes. Acute respiratory distress syndrome (ARDS) is a serious problem affecting the lungs, especially in the intensive care unit, and is caused by a wide variety of triggering factors. Although very different, both illnesses share diagnostic problems which can lead to delays in diagnosis. These delays can, in turn, delay interventions which could change the course of the disease.

Metabolomics is one of the 'omics' fields, where modern analytical methods allow for the measurement of multiple metabolites simultaneously, and in combination with multivariate data analysis, can identify metabolites significantly affected by a disease. In this work, nuclear magnetic resonance spectroscopy (NMR) -based metabolomics was used to investigate CKD, by comparing a rat model of CKD to sham-operated rats, and ARDS, by investigating patients with low blood oxygen levels (hypoxaemia) after cardiac surgery with cardiopulmonary bypass.

Significant findings suggest that oxidative stress and energy metabolites are affected in both illnesses. In CKD, the gut-kidney axis and acid-base balance also seem to be affected, whilst in post-operative hypoxaemia, inflammation and the stress response could explain some of the metabolite changes. Complicated interactions between metabolites and metabolite pathways, and underlying methodological issues, mean that results need to be investigated further and validated using larger groups and different methods.
